Planning, Counseling and Support
 Starting on the day of admission and
through every day of a resident’s stay with
us, both family and resident can count on counseling
and support from our trained social service staff.
Caring Connections assigns a staff
member to each family when their loved
one is admitted. This Caring Connections
representative serves as a liaison to
solve any problems that might arise
during a resident’s stay. The
representative keeps the family up-to-date
monthly on their loved one’s progress
and discusses any issues the family
may need to resolve. By encouraging
open communication between facility
and family members, Caring Connections
enables us to provide better service
and satisfaction in care.
Taking that commitment to excellence
even further, all of our administrative
and patient care staff are trained to
be “Committed to Caring.” Our
comprehensive training program encourages
every staff member to focus on the unique
needs of you and your loved one while
delivering quality service.
We give careful consideration to the
appropriate selection of roommates,
with personalities and individual needs
as guidelines.
Careful discharge planning assures
a smooth transition to independent living.
To assist our residents in adjusting
to life after leaving our facility,
we notify community resources and agencies
that may be able to help. We also share
this information with the family and/or
resident.
